Either the patient requests a review or assessment, or a member of practice staff identifies a review is due.
A customised link, either using the practice website domain, text message, a patient portal or a YIHA (Your Intelligent Health Assessment) address, is sent to the patient.


The patient completes the online assessment / review / admin form in their own time at their own pace.
The information is organised into a handy PDF file which is emailed to the practice to be acted upon as you see fit. Alternatively, it is transferred to the clinician via our MEDTECH integration and placed in the patient’s inbox plus added to the daily record while also updating the patient dashboard.
