Either the patient requests a review or assessment, or a member of practice staff identifies a review is due.
A customised link, either using the practice website domain, text message, a patient portal or a YIHA (Your Intelligent Health Assessment) address, is sent to the patient.


The patient completes the online assessment / review / admin form in their own time at their own pace.
The information is organised into a handy PDF file which is emailed to the practice to be acted upon as you see fit. Alternatively, it is transferred to the clinician via our MEDTECH integration.

Patient privacy comes first
We will never sell or share any information that could identify an individual patient.
Your practice is always in control
If your practice’s name ever needs to be included in reporting (for example, for PHO quality management or benchmarking), we will only do this with your explicit written consent.
Data used for good
We use anonymised and aggregated data to help identify and address inequities in healthcare. This allows us to support:
- Targeted equity programmes that reach the patients and communities who need them most
- Better understanding of outcomes across areas, ethnicities, age groups and genders
- Insights that help practices, PHOs and funders design support where it will have the greatest impact
Anonymous insights, shared responsibly
Aggregated, anonymised insights may be shared with PHOs, government agencies, and other health partners — but always in a form that protects your patients and your practice.

We comply with the Privacy Act 2020, the Health Information Privacy Code, and other data protection laws. More importantly, we commit to using data in ways that benefit patients, practices and the wider health system.
