Either the patient requests a review or assessment, or a member of practice staff identifies a review is due.
A customised link, either using the practice website domain or a YIHA (your intelligent health assessment) address, is sent to the patient.
The patient completes the online assessment / review / admin form in their own time at their own pace.
The information is processed and organised into a handy PDF file which is emailed to the practice to be acted upon as you see fit.
